Experienced Community Physiotherapist - Aged Care

Help older Australians stay independent, active and confident in their own homes. Right at Home Gold Coast is seeking a passionate and motivated Physiotherapist to join our growing community aged care team. This is an opportunity to take your clinical expertise beyond the traditional clinic environment and work directly with older clients in their own homes and communities.

Responsibilities

Completing comprehensive physiotherapy and functional assessments; Developing individualised treatment and rehabilitation plans; Supporting mobility, strength, balance and functional independence; Falls risk assessment, prevention and management; Prescribing appropriate exercise and home-based rehabilitation programs; Assessing mobility and recommending appropriate aids or equipment; Supporting clients living with chronic health conditions and age-related changes; Educating clients, families and Care Partners about safe mobility and exercise; Monitoring client progress and adjusting interventions as required; Working collaboratively with Nurses, Care Partners, GPs, allied health professionals and families; Maintaining accurate clinical documentation and care plans; Identifying changes in client health or function and escalating concerns appropriately

Requirements

Bachelor's or Master's degree in Physiotherapy; Current AHPRA registration; Current Australian driver's licence; Strong clinical reasoning and assessment skills; Excellent communication and interpersonal skills; The ability to work independently within clients' homes; A commitment to person-centred and evidence-based care; An interest in aged care, rehabilitation, healthy ageing and maintaining independence
