In-home Traveler NP or PA (Full-Time) - New York State and Surrounding Markets

Signify Health, part of CVS Health, is seeking a Full-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. In this role, you'll be primarily traveling - up to 100% of the time - across a defined coverage area, as defined by business needs. This may require overnight hotel stays, car rentals and/or flights, depending on the prescribed coverage area. The visit, which lasts about 45 minutes, allows for time to connect one-on-one and answer health questions. It includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests. You’ll use an iPad and an intuitive clinical workflow for each evaluation. Responsibilities

Provide In-Home Health Evaluations, engaging with people in the comfort of their own homes; Perform 45-minute visits including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered by the health plan; Use an iPad and intuitive clinical workflow for each evaluation; Travel locally and within licensed states, up to 100% of the time; Conduct virtual visits as needed; Provide all Signify Health services including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.); Join a national network of purpose-driven clinicians dedicated to improving health outcomes

Requirements

Active, unrestricted license(s) in coverage area(s); Board certification; Able to obtain and maintain multi-state licenses in coverage area, as required; Able to cover all counties within New York and flex to other areas of business need; New York State Coverage Areas include Albany, Binghamton, Buffalo, Elmira, NYC, Plattsburgh, Poughkeepsie, Rochester, Syracuse, Utica and Watertown; Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants; Flexible to travel locally and within licensed states, up to 100% of the time
