In-home Traveler Physician (Full-Time) - State of New Hampshire and Surrounding Markets

CVS Health/Signify Health is seeking a Full-time Physician to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. In this role, you'll be primarily traveling up to 100% of the time across a defined coverage area. The visit lasts about 45 minutes, includes a medication and medical history review, a physical evaluation, and additional diagnostic tests if ordered. You'll use an iPad and an intuitive clinical workflow for each evaluation.

Responsibilities

Provide In-Home Health Evaluations including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered; Engage with people in the comfort of their own homes and help bridge gaps in care; Travel locally and within licensed states, up to 100% of the time; Conduct virtual visits as needed; Use an iPad and intuitive clinical workflow for each evaluation; Provide all Signify Health services including Diagnostic & Preventive Services products (spirometry, DEE, etc.)

Requirements

MDs and DOs; Primary residence in the state of New Hampshire; Active, unrestricted license(s) in coverage area(s); Able to obtain and maintain multi-state licenses in coverage area, as required; Able to cover all counties within New Hampshire and flex to other areas of business need; Flexible to travel locally and within licensed states, up to 100% of the time; Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
