Registered Nurse - Home Health Case Manager (Sugar Grove, Yorkville and surrounding)

Registered Nurse - Home Health. Location: Sugar Grove, Yorkville, Plano, Bristol, Big Rock and surrounding areas. Hours: M-F 8am to 5pm job with weekend and holiday rotations. Every fourth weekend rotation. One winter and one summer holiday. On call rotation 3 to 4 times a month. This position is eligible for up to a $10,000.00 sign on bonus.

Responsibilities

Develops and maintains a therapeutic relationship throughout the healthcare continuum; Provides individualized care to promote physical and psychosocial comfort; Assesses patient and family readiness and involves them in planning and implementing care; Assures/improves quality care and mobilizes appropriate resources; Facilitates patient and family's right to receive quality, cost effective care; Adapts practice to latest standards; Participates in quality/process improvement initiatives and patient safety efforts; Meets regulatory and mandatory requirements; Educates patients and families and supports learning needs; Uses nursing process as a problem-solving model; Recognizes changes in clinical situations and implements changes to plan of care; Prioritizes and organizes patient care; Initiates referrals and discharge planning; Applies ethical decision making; Monitors confidentiality and assures appropriate business conduct; Delegates correctly to unlicensed assistive personnel; Demonstrates leadership and collaborates with healthcare team; Participates in unit meetings and activities; Utilizes available resources to maintain knowledge; Achieves required certifications or Bachelor's Degree; Earns minimum 10 contact hours per year

Requirements

Nurse, Registered (RN); Current Illinois RN license; Current CPR; Graduate from an accredited school of nursing; Earn 10 continuing education credits annually; One year of medical surgical experience; Demonstrated proficiency for the requirements for RN role; Good communication and interpersonal skills; Good documentation skills; Computer skills; Good time management and organizational skills; Familiarity with patient care equipment; Ability to work weekends, holidays, and different shifts; Ability to take on call assignments; Ability to handle multiple tasks and complete work within short timeframes; Ability to transport objects, patients, and equipment; Ability to respond quickly to patient needs and work at fast pace; Ability to travel to all areas within the Advocate service area in all extremes of weather on all types of roads; Must be willing to work occasional overtime if needed; Required to make visits to private residences which may not be accessible to people with disabilities; Ability to assist with ambulation including turning and transferring patients up to 200 pounds; Ability to lift up to 35 pounds without assistance
